Fill out the form with the information used to sign up for Planet Fitness.

Personalized Cancellation Letter
Quitness will automatically generate a personalized cancellation letter addressed and signed to your home Planet Fitness club.

Cancellation Letter Sent via First Class Certified Mail
Quitness sends your personalized letter via USPS First Class Certified Mail to your home Planet Fitness club. We'll provide tracking info so you can track your cancellation letter along the route.

Membership Cancellation
Your cancellation letter will be hand delivered to the front desk at your Planet Fitness home club. From here, they're required to cancel your membership upon receipt. You should also receive an email from Planet Fitness notifying you that your cancellation has been processed.
